    Dr. Hae Mi Choe is a Chief Population Health Officer (CPHO) at University of Michigan Health. She also serves as an Associate Dean and Clinical Professor at University of Michigan College of Pharmacy. At the state level, Dr. Choe serves as the Executive Director for Michigan Institute for Care Management & Transformation (MICMT). She collaborates with payers and physician organization leadership to establish strategic direction and implementation plan for provider-delivered care management and team-based care model across the state. Dr. Choe received numerous state and national recognition for innovations in clinical practice and leadership. She has focused her research efforts on improving chronic disease management and care delivery models. Dr. Choe earned her PharmD from University of California, San Francisco, and completed her pharmacy residency with Kaiser Permanente and a health care policy fellowship at the Center for Healthcare Research and Transformation in Ann Arbor, MI.

    Director of Ambulatory Clinical Pharmacy Practices at the U-M Medical Group, Amy Thompson came to the University of Michigan College of Pharmacy from the South Carolina College of Pharmacy/Medical University of South Carolina (SCCP/MUSC). She is a graduate of the MUSC College of Pharmacy and also completed PGY1 and PGY2 residencies specializing in family medicine. After completing her residency, she joined the faculty at SCCP/MUSC and held a clinical practice site in an internal medicine clinic since 2007. Thompson is a Board Certified Ambulatory Care Pharmacist and Certified Diabetes Educator. In addition to clinical practice, Thompson has served as a residency program director for the PGY1/PGY2 Ambulatory Care pharmacy residency at MUSC. Her research interests include geriatrics, interprofessional education, medication safety and transitions of care.
