Originally from Kennewick, Washington, Dr. Emily Ashjian earned her Bachelor of Science in Molecular, Cellular and Developmental Biology at the University of Washington and her Pharm.D. at the University of North Carolina Eshelman School of Pharmacy. She completed her PGY1 Pharmacy Practice Residency and PGY2 Ambulatory Care Pharmacy Residency at UMHS, where she also served as the PGY1 and PGY2 Chief Resident. Ashjian’s professional interests include diabetes, cardiovascular risk reduction, transitions of care, health literacy and the development and advancement of new and innovative ambulatory care pharmacy services. In her free time, Ashjian enjoys running, swimming, traveling and spending time with family and friends.

Anisa Bici grew up in Southgate, Michigan. She earned her Bachelor of Science in Biology from the University of Michigan-Dearborn. Anisa graduated from the University of Michigan with a Doctor of Pharmacy Degree in 2020 and completed her PGY-1 Pharmacy Practice Residency at Michigan Medicine. Her professional and academic interests include cardiology, anticoagulation, teaching and mentoring student pharmacists. Outside of pharmacy, Anisa enjoys reading, playing tennis and spending time with family and friends.

Carly Burns earned her Doctor of Pharmacy Degree from the Eugene Applebaum College of Pharmacy and Health Sciences at Wayne State University. She completed her PGY1 Pharmacy Practice Residency at Ascension St. John Hospital and PGY2 Ambulatory Care Residency at the Detroit Medical Center. Carly currently works as a PCMH clinical pharmacist and provides pharmacy services at the East Ann Arbor Health Center and the Chelsea Health Center. Her professional interests include diabetes and geriatric management.

Hae Mi Choe is a clinical associate professor in the Department of Clinical Pharmacy at the University of Michigan College of Pharmacy and the director of Innovative Ambulatory Pharmacy Practices. Choe earned her PharmD in 1993 from the University of California, San Francisco, and in 1994 completed her pharmacy practice residency with Kaiser Permanente in Los Angeles. She spent five years overseas, where she introduced, developed, and implemented anticoagulation and asthma services at the Samsung Medical Center in Seoul, South Korea. Choe then joined U-M and developed ambulatory clinical services at the East Ann Arbor Health Center. In her leadership role as Director of the Innovative Ambulatory Pharmacy Practices, Choe, along with her colleagues, created a group-practice model at 14 sites within the U-M Hospitals and Health Centers as part of the “patient-centered medical home” initiative. She also developed a new reimbursement process for clinical pharmacy services. Choe was the first pharmacist endorsed by the Credentialing Committee at the U-M Health System and was granted special privileges in patient care; she has led her entire clinical pharmacist team through the credentialing process.

Heidi Diez grew up in Ann Arbor, and earned her Doctor of Pharmacy degree from the U-M College of Pharmacy. She completed a PGY1 Community Practice Residency at the University of Washington/Kelley-Ross site in 2005. In 2007, Diez joined the faculty at the College of Pharmacy, in a newly created shared Community Pharmacy position. She later transitioned her clinical practice to Ambulatory Care, joining the team of UMHS PCMH pharmacists. Since joining the PCMH group, Diez—the first pharmacist in Family Medicine—has established PCMH Pharmacy services at four sites across UMHS. Currently, she provides care at Briarwood Family Medicine and Domino’s Family Medicine—and in a new role, serves as a Process Improvement Pharmacist for Michigan Pharmacists Transforming Care and Quality (MPTCQ). Her research interests include diabetes, patient adherence, immunizations, bridging care between primary care and community practice and innovative teaching modalities. In her free time, she enjoys cycling, U-M football and hockey games, traveling, new restaurants and spending time with family and friends.

Nicholas Dillman received his Doctor of Pharmacy degree from the University of Michigan College of Pharmacy before completing a PGY1 in Pediatrics and a PGY2 in Pediatrics with a focus in Antimicrobial Stewardship/Infectious Diseases at Texas Children’s Hospital. He returned to Michigan Medicine in 2016 in an inpatient Antimicrobial Stewardship/Infectious Diseases role before transitioning to Ambulatory Antimicrobial Stewardship in 2021. Nick’s professional interests include ensuring appropriate antimicrobial prescribing practices, minimizing the impact of antimicrobial use on patients, health systems, and communities, and leveraging technology to improve access to data.

Mary Jo Elder grew up in Madison, WI and earned her Doctor of Pharmacy degree at Drake University in Des Moines, IA. She completed a PGY1 Community Pharmacy Residency and PGY2 Ambulatory Care Pharmacy Residency at Michigan Medicine. She currently serves in multiple areas as a clinical pharmacist including at Briarwood Family Medicine, Central Blood Pressure Support clinic, and the Anticoagulation Outpatient Service. Her professional interests include diabetes and hypertension management, anticoagulation, and precepting learners. Outside of pharmacy, Mary Jo enjoys baking, reading, traveling, and spending time with family and friends.

Nada Farhat grew up in Dearborn, MI. She earned her Doctor of Pharmacy degree from Wayne State University’s Eugene Applebaum College of Pharmacy. Nada completed her PGY1 Pharmacy Practice Residency and PGY2 Ambulatory Care Pharmacy Residency at Michigan Medicine. She currently practices in multiple areas as a clinical pharmacist including at the Saline Health Center and within the Anticoagulation Outpatient Service. Additionally, she serves as a clinical lead for the Michigan Institute for Care Management and Transformation. Through this role, Nada helps strengthen patient engagement throughout the state of Michigan by providing support to Physician Organizations and other healthcare professionals working on collaborative quality initiatives. Outside of pharmacy, Nada enjoys baking, yoga, and spending time with her family and friends.

Jessica Fennelly grew up in Milan, MI. She received her Bachelor of Science in Biology from the University of Michigan (UM), and then earned a Doctor of Pharmacy degree from UM College of Pharmacy. After college, she completed a PGY1 Community Pharmacy Residency and PGY2 specialty residency in Ambulatory Care at Michigan Medicine. Fennelly joined the team of Michigan Medicine PCMH pharmacists in 2017. In her role currently, she provides primary care clinical services and engages in quality improvement initiatives at the Canton Health Center. She also provides care as a part of the transitions of care team of pharmacists. In her spare time, Fennelly enjoys spending time with her family, friends, and chocolate lab. She is also passionate about wellness, loves to travel, and spend time outdoors.

Ben is from Plymouth, Michigan. He earned his bachelor’s degree in Biomolecular Science from the University of Michigan (Ann Arbor) and liked it so much he stayed an extra four years to get his Doctorate of Pharmacy from there too. He then completed his PGY-1 at Corewell Health Hospital in Dearborn, Michigan and is now back at U of M for his PGY-2 in ambulatory care. His professional interests include ambulatory care and infectious disease. He hopes to work in an ambulatory clinic following his residency. When he’s lucky enough to have free time, Ben enjoys bowling, tennis, gaming, and spending time with his friends.
