    Hae Mi Choe is a clinical associate professor in the Department of Clinical Pharmacy at the University of Michigan College of Pharmacy and the director of Innovative Ambulatory Pharmacy Practices. Choe earned her PharmD in 1993 from the University of California, San Francisco, and in 1994 completed her pharmacy practice residency with Kaiser Permanente in Los Angeles. She spent five years overseas, where she introduced, developed, and implemented anticoagulation and asthma services at the Samsung Medical Center in Seoul, South Korea. Choe then joined U-M and developed ambulatory clinical services at the East Ann Arbor Health Center. In her leadership role as Director of the Innovative Ambulatory Pharmacy Practices, Choe, along with her colleagues, created a group-practice model at 14 sites within the U-M Hospitals and Health Centers as part of the “patient-centered medical home” initiative. She also developed a new reimbursement process for clinical pharmacy services. Choe was the first pharmacist endorsed by the Credentialing Committee at the U-M Health System and was granted special privileges in patient care; she has led her entire clinical pharmacist team through the credentialing process.

    Director of Ambulatory Clinical Pharmacy Practices at the U-M Medical Group, Amy Thompson came to the University of Michigan College of Pharmacy from the South Carolina College of Pharmacy/Medical University of South Carolina (SCCP/MUSC). She is a graduate of the MUSC College of Pharmacy and also completed PGY1 and PGY2 residencies specializing in family medicine. After completing her residency, she joined the faculty at SCCP/MUSC and held a clinical practice site in an internal medicine clinic since 2007. Thompson is a Board Certified Ambulatory Care Pharmacist and Certified Diabetes Educator. In addition to clinical practice, Thompson has served as a residency program director for the PGY1/PGY2 Ambulatory Care pharmacy residency at MUSC. Her research interests include geriatrics, interprofessional education, medication safety and transitions of care.
