Lianne Granata, Pharm.D.
Clinical Pharmacist Specialist - Ambulatory Clinical Pharmacy Practice734-232-2497granatal@med.umich.eduLianne Granata is originally from Michigan and earned her Doctor of Pharmacy Degree from the Eugene Applebaum College of Pharmacy and Health Sciences at Wayne State University. She completed both her PGY1 Pharmacy Practice Residency and PGY2 Ambulatory Care Residency at the Detroit Medical Center. Lianne currently works as a PCMH clinical pharmacist and provides primary care disease state management services out of the Northville and Livonia Health Centers as well as central transitions of care services for patients recent discharged from UMHS. She also runs the department’s Medication Optimization program which aims to improve medication use across the health system. Her professional interests include hypertension, weight management and complementary and alternative medicine. Her interests outside of pharmacy include spending time with her son and animals, reading, gardening, learning French and traveling. She has been to all 50 states and one day hopes to be able to say she’s visited every country in the world.

Nghi B. Ha, Pharm.D., MPH, BCPS, CACP
Clinical Pharmacist Specialist – Ambulatory Cardiology and Anticoagulation Management – Michigan Medicine; Adjunct Clinical Assistant Professor – University of Michigan College of Pharmacy734-232-6853nghih@umich.eduNghi Ha is originally from Sunnyvale, California. He earned his Bachelor of Science in Genetics from the University of California, Davis. After graduating, he spent three years conducting clinical research specializing in the epidemiology and treatment of hepatitis B, hepatitis C and hepatocellular carcinoma in Asian American patients, while concurrently earning a Master of Public Health degree. Ha graduated from the University of California, San Francisco School of Pharmacy with a Doctor of Pharmacy degree in 2014, and completed his PGY-1 Pharmacy Practice Residency at the University of Michigan Health Systems. His professional and academic interests include cardiology, anticoagulation in special populations, health disparities and evaluation of pharmacists’ impact on improving patient care. He is also passionate about teaching and mentoring student pharmacists and residents. In his free time, he enjoys photography, spending time with friends, training his dog and exploring new places and restaurants.

Marcy Holler, Pharm.D., BCPS
Clinical Pharmacist Specialist - Ambulatory Clinical Pharmacy Practice248-305-4405mtdm@umich.eduOriginally from Ann Arbor, Marcy Holler received her bachelor’s degree in Biology from Northwestern University and her Doctor of Pharmacy degree from the U-M College of Pharmacy. She became a Board Certified Pharmacotherapy Specialist in 2010 and completed postgraduate residency training at both the U-M (PGY1) and UNC (PGY2-Ambulatory Care) Health Systems. After completing her training, Holler worked for a year as a clinical pharmacist practitioner in a new primary-care clinic built as part of a joint collaboration between UNC Hospital and BlueCross BlueShield of North Carolina. In 2012, she accepted a co-funded Clinical Assistant Professor position with Henry Ford Hospital and Wayne State University, teaching and providing clinical services in the K15 Internal Medicine clinic. Holler returned to her hometown in 2014 and (re)joined the University of Michigan Health System, providing chronic disease management services at clinics in Northville and Ann Arbor. Holler’s clinical and professional interests include chronic disease management, patient education, medication adherence, preventive healthcare and patient empowerment. In her free time she enjoys reading, listening to music, traveling and spending time with her family.

Madison Irwin, Pharm.D., BCPS
Clinical Pharmacist Specialist; - Palliative Care; Clinical Assistant Professor - College of Pharmacymadisoir@umich.eduMadison Irwin is a Clinical Assistant Professor at the University of Michigan College of Pharmacy and a Clinical Pharmacist Specialist in Palliative Care at University of Michigan Health. She graduated with her Bachelor of Science in Biology from the University of Dayton in 2015 and her Doctor of Pharmacy from the Ohio State University College of Pharmacy in 2019. She subsequently completed PGY-1 Pharmacy Practice and PGY-2 Pain Management and Palliative Care residencies at University of Michigan Health. Following residency, Irwin completed a two-year Clinical and Translational Sciences Fellowship at the University of Michigan College of Pharmacy under the supervision of Dr. Michael Smith. In her current role, she supports the outpatient palliative care clinics and the inpatient palliative care consult team at University of Michigan Health. Her clinical practice and research focus on novel pharmacy practice initiatives to optimize the care of vulnerable populations. Areas of interest include the intersection of insomnia, pain and symptom management, and medication utilization trends in palliative care populations.

Kippes currently works as a PCMH clinical pharmacist and provides pharmacy services at Dexter Health Center and Saline Health Center. She also serves as a clinical pharmacist for central transitions of care at UMHS. Her professional interests include diabetes care and management and transitions of care. Interests outside pharmacy include traveling, spending time with family and friends and cooking.

Valerie Lindell, Pharm.D., BCACP
Clinical Pharmacist Specialist - Ambulatory Clinical Pharmacy Practice; Adjunct Clinical Instructor of Pharmacy - College of Pharmacyvpalmer@umich.eduValerie Lindell grew up in Wheeling, WV and earned her Doctor of Pharmacy degree at West Virginia University College of Pharmacy in Morgantown, WV. She completed her PGY-1 Pharmacy Practice Residency at Henry Ford Hospital Main Campus in Detroit, and her PGY-2 Ambulatory Care Pharmacy Residency at the University of Michigan Health System in Ann Arbor. Lindell is pursuing her career as a PCMH clinical ambulatory specialty pharmacist at the University of Michigan, providing primary care services out of the Brighton Health Center & specialty care services out of the Brighton Center for Specialty Care. She also is engaged in central transitions of care practices, Chelsea Hospital heart failure transitions of care practices, and ambulatory innovative pharmacy projects. She trains Meijer PharmDs to manage hypertension in Meijer stores via the Meijer Hypertension Program Partnership with Michigan Medicine. Interests outside of pharmacy include weightlifting, running, and calisthenics. Lindell also enjoys spending time with family and friends, her husband and dog, and traveling the world.

Beatriz Manzor Mitrzyk, Pharm.D., BCPS, BCACP
Clinical Assistant Professor and Assistant Research Scientist734-615-5278bmitrzyk@med.umich.eduOriginally from Grosse Pointe Woods, MI, Dr. Beatriz Manzor Mitrzyk earned her Doctor of Pharmacy degree at the University of Michigan College of Pharmacy (UM COP) and completed a Pharmacy Practice Residency at UMHS. She has worked as a Clinical Specialist in Internal Medicine and Pharmacokinetics at Detroit Receiving Hospital as well as a Clinical Specialist in Infectious Diseases at Sinai Hospital. She was also the Regional Director for Clinical Practice Assessments at the University of Florida College of Pharmacy Working Professional PharmD Program and worked as a freelance medical writer writing continuing education for pharmacists. Most recently, she completed a Postdoctoral Research Fellowship at the UM COP and is now a Clinical Assistant Professor and Assistant Research Scientist at UM COP. She is an Ambulatory Clinical Pharmacist in the Saline Health Center PCMH and conducts research to address medication adherence-related health disparities in Latinx persons with mental health disorders. She also enjoys traveling, reading, gardening, exercising, and spending time with her family and friends.

Rima Mohammad, Pharm.D., BCPS, FCCP
Clinical Pharmacy Specialist and Clinical Associate Professor in Pharmacy - College of Pharmacyrimam@med.umich.eduRima A. Mohammad is a Clinical Associate Professor of Clinical Pharmacy at the University of Michigan College of Pharmacy and a Post-ICU and Hepatology Clinical Pharmacist Specialist at Michigan Medicine. She received her Doctor of Pharmacy from The James L. Winkle College of Pharmacy, University of Cincinnati. She completed her pharmacy practice and critical care specialty residency at Michigan Medicine. She is board certified specialist in pharmacotherapy and a fellow of the American College of Clinical Pharmacy (FCCP).
At Michigan Medicine, she is a key member of the interprofessional team in an innovative clinical model (the University of Michigan, Post ICU Longitudinal Survivor Experience [PULSE] clinic) in caring for ICU survivors and caregivers. She is also a key member of the hepatology outpatient clinic in providing care to patients with liver cirrhosis during transitions of care after being discharged from a hospital.
Her research focuses on the areas of internal medicine, critical care and gastroenterology/hepatology. Her passion and primary goal is improving patient outcomes through various drug therapy approaches and innovative practice models and services. Most of her scholarly work has focused on linking her significant efforts in clinical service to making an impact at all levels of practice including local, national, and international levels. Key examples of this approach include her significant involvement in several clinical practice and transitions of care initiatives. She is committed to continue her involvement and focusing on developing and implementing innovative clinical services to improve patient outcomes. -
Erin Mouland, Pharm.D., CACP

She completed her PGY-1 Pharmacy Practice Residency at the Veterans Affairs Ann Arbor Healthcare System with a focus in ambulatory care. Mouland is a Clinical Pharmacist in the Anticoagulation Outpatient Service at Domino’s Farms in Ann Arbor. Some of her interests outside of pharmacy include spending time with her family and yoga.

Hanna Phan, Pharm.D., FCCP, FPPA
Clinical Associate Professor - College of Pharmacy; Clinical Pharmacist Specialist - Ambulatory Clinical Pharmacy Practice734-647-1629hphan@med.umich.eduHanna Phan is a Clinical Associate Professor at the U-M College of Pharmacy and Clinical Pharmacist Specialist for the Pediatric Cystic Fibrosis Center at C.S. Mott Children’s Hospital – Michigan Medicine. She earned her Doctor of Pharmacy degree from the U-M College of Pharmacy and completed Postdoctoral Fellowship in Pediatric Pharmacotherapy at The Ohio State University/Nationwide Children’s. Prior to her return to Ann Arbor, Phan was part of the faculty at the University of Arizona College of Pharmacy in Tucson, AZ for 12 years, having served as Assistant Department Head and Associate Professor. During her tenure at the University of Arizona, Phan developed the first PGY2 Pediatric Pharmacy residency, served as residency program director for several years, developed innovative clinical pharmacy services in pediatric pulmonary medicine, with a focus on cystic fibrosis and asthma, as well as lead and contributed to clinical research regarding medication adherence in cystic fibrosis and asthma. Currently, she is developing clinical pharmacy services, including integration of a pharmacy technician, as part of an interprofessional care model in pediatric cystic fibrosis. Her research interests include medication adherence, patient/family education, coproduction of care for children and youth with special health care needs, and interprofessional care models. In her free time, she enjoys photography, running, U-M football and basketball games, exploring local cuisine, and traveling.
