Pharmacy Innovations and Partnerships manages a number of new programs at the U-M Health System that include:
- Brighton Center for Specialty Care (BCSC)
- University of Michigan Health Chronic Kidney Disease Service
- University of Michigan Health Clinical Pharmacy Blood Pressure Service
- University of Michigan Health Anticoagulation Service
- Antimicrobial Stewardship Program (ASP)
- University of Michigan Health Psychiatry Service
- University of Michigan Health Transitions of Care Services (TOC)
- University of Michigan Palliative Care Service
- University of Michigan Health Patient-Centered Medical Homes
If successful, these programs can be developed to expand across our own organization and be shared with our partnering POM ACO organizations and/or through Michigan Pharmacists Transforming Care and Quality.
