Outpatient Palliative Care Pharmacy Services at the University of Michigan provide support for patients and providers in the outpatient palliative and supportive care clinics within the institution. This referral-based, pharmacist-led service is available to established patients of these clinics. Services are provided by specialty trained palliative care clinical pharmacists in close collaboration with providers, nurses, social workers, and other members of the healthcare team. We provide transitions of care services for patients receiving palliative care, assistance with complex medication management, and medication reviews tailored to each patient’s unique needs. All services are delivered virtually to improve accessibility to patients across the state of Michigan. Educational opportunities are available to pharmacy residents and students to foster the development of expertise in palliative care management among future pharmacy practitioners.
