University of Michigan Health Chronic Kidney Disease Service

The multidisciplinary Chronic Kidney Disease (CKD) Clinic at University of Michigan Health is an innovative clinic that includes two nephrologists, a clinical pharmacist, renal dietician, and social worker. Team members in this clinic work collaboratively to provide care for complex patients living with CKD with the goal of slowing progression of their CKD and delaying the time until renal replacement therapy is needed. The clinical pharmacist completes a comprehensive medication review with each patient seen, with a focus on the management of hypertension, diabetes, and dyslipidemia in the setting of chronic kidney disease as well as renal dosing, medication adherence, and patient education. The multidisciplinary CKD clinic supports pharmacy learners at multiple levels, with rotations available for P4 APPE students and PGY1 and PGY2 pharmacy residents.
