University of Michigan Health Patient-Centered Medical Homes

At the University of Michigan Patient-Centered Medical Homes, ambulatory care pharmacists play an integral role in delivering patient-centered care for individuals managing Type 2 Diabetes, hypertension, dyslipidemia, and tobacco use disorder. Our team collaborates with a multidisciplinary team consisting of physicians, nurse practitioners, physician assistants, dietitians, social workers, and nursing staff to ensure comprehensive and holistic care for patients. Operating under collaborative practice agreements, pharmacists have the authority to initiate, adjust, and discontinue pharmacotherapy for these chronic conditions. In addition to medication management, pharmacists offer drug information services and assist patients in overcoming medication access barriers within the primary care setting. Furthermore, our program provides educational opportunities for IPPE and APPE students, as well as PGY1 and PGY2 pharmacy residents, enabling the development of expertise in ambulatory care pharmacy practice.
