The University of Michigan Anticoagulation Service offers specialized care to patients institution wide to mitigate thrombotic and bleeding complications associated with antithrombotic therapy. Our services are facilitated utilizing collaborative drug therapy management through which we provide comprehensive anticoagulation management, with a focus on warfarin and direct oral anticoagulant therapy. The clinical pharmacy team works in collaboration with providers and nurses to ensure the highest standard of care. Additionally, we collaborate with inpatient anticoagulation services to provide seamless transitions of care for patients. The anticoagulation service offers educational opportunities for IPPE and APPE students, as well as PGY1 and PGY2 pharmacy residents, fostering the development of expertise in anticoagulation management.
